NAD+ Boosters and Aging: What the Science Actually Says
NAD+ boosters like NR and NMN reliably raise a number in your blood. Whether that slows aging is a much shakier claim than the marketing suggests.
NAD+ (nicotinamide adenine dinucleotide) is a molecule your cells use to turn food into energy and to run hundreds of basic chemical reactions. Over the past decade it has become the centerpiece of a booming longevity-supplement market. The pitch is simple and seductive: NAD+ falls as you get older, so topping it up with precursors like nicotinamide riboside (NR) or nicotinamide mononucleotide (NMN) should slow aging. The reality, once you read the actual human trials, is far more modest and far less settled. This explainer walks through what the evidence does and does not support.
Does NAD+ even decline with age?
This is the foundation of the entire NAD+ story, and it is genuinely contested. Much of the 'NAD+ declines with age' narrative rests on studies in rodents and a limited number of tissue-specific human studies. A 2026 study in Nature Metabolism measured whole-blood NAD+ across seven independent human cohorts using validated mass spectrometry and found that levels stayed remarkably stable with age and were unchanged by lifestyle interventions. They did rise with nicotinamide riboside supplementation, but the authors went so far as to challenge the use of blood NAD+ as a biomarker of aging at all.
An authoritative 2025 Nature Metabolism review reached a similar conclusion, noting that a consistent age-related decline in NAD+ in humans has been observed in only a limited number of studies. It is also worth keeping in mind that blood NAD+ and tissue NAD+ are not the same thing. Even if levels fall in muscle, brain, or liver with age, a blood measurement may not reflect what is happening inside those tissues, and vice versa.
What NAD+ boosters reliably do
There is one finding that is robust and reproducible: oral NR (and NMN) raises the amount of NAD+ circulating in your blood, and it does so in a dose-dependent way. In a randomized, double-blind, placebo-controlled trial of 140 overweight adults, NR at 100, 300, and 1000 mg per day increased whole-blood NAD+ by roughly 22%, 51%, and 142% respectively by day 14. It appeared safe over the short term, with no significant difference in adverse events versus placebo.
But raising a number in your blood is not the same as improving your health. In that same trial, NR did not significantly change LDL cholesterol, homocysteine, C-reactive protein (a marker of inflammation), or resting energy expenditure. The supplement did what it says on the tin, biochemically, without moving the metabolic markers people hope it will.
The single most reproducible finding is narrow: these supplements raise blood NAD+. Whether that helps a healthy person is a separate, largely unproven question.
Do the benefits actually follow?
This is where the hype and the evidence part ways. The 2025 review put it bluntly: while animal studies make NAD+ precursors look promising, human clinical trials have shown limited efficacy. The human results are best described as small, inconsistent, or null. Consider the trials most often cited on each side:
- The metabolic 'win'A small randomized trial found that NMN at 250 mg per day for 10 weeks improved insulin-stimulated glucose disposal in the skeletal muscle of postmenopausal women with prediabetes. But the trial was small, other metabolic outcomes did not improve, researchers could not detect an actual rise in muscle NAD+, and the study drew published methodological criticism.
- Healthy older menNMN at 250 mg per day for 12 weeks raised blood NAD+ and produced only modest gains in gait speed and left-hand grip strength, with no effect on muscle mass, body composition, insulin sensitivity, cognition, or blood pressure.
- CognitionIn older adults with mild cognitive impairment, NR escalated to 1 g per day for about 10 weeks raised blood NAD+ roughly 2.6-fold, yet cognitive test scores did not improve over placebo. Getting more NAD+ into the blood did not translate into a benefit even where you would most hope to see one.
- Lifespan in animalsIn the gold-standard test, the NIH's Interventions Testing Program fed NR to genetically diverse mice and found no effect on median or maximum lifespan in either males or females.
Taken together, the pattern is consistent: the biochemistry moves, the outcomes mostly do not. Where benefits appear, they are small and do not reproduce cleanly across studies or outcomes.
Safety, regulation, and honest caveats
On short-term safety, the news is reassuring. High doses appear well tolerated: the NR-SAFE trial gave 20 Parkinson's patients 3000 mg of NR per day for four weeks and reported no moderate or severe adverse events. That was a Phase I safety study, not proof of efficacy, and the researchers flagged that an apparent clinical improvement was potentially confounded. Reassuring on safety is not the same as proven to work.
The regulatory picture shows how new and commercially driven this category is. NMN's U.S. status has whipsawed: in November 2022 the FDA concluded NMN could not be sold as a dietary supplement, then in September 2025 reversed course and determined it is not excluded. That decision is only about whether NMN can be legally sold as a supplement. It is not a judgment on whether NMN works or is safe long-term. Because dietary supplements are not FDA-approved before sale, product quality and label accuracy also vary from brand to brand.
The bottom line
If you strip away the marketing, here is what the science actually says. NAD+ precursors reliably raise NAD+ in your blood and look safe over weeks-to-months in the trials done so far. Beyond that, almost everything is uncertain: it is not even clear that blood NAD+ meaningfully declines with age in humans, and boosting it has not been shown to reliably improve muscle, metabolism, cognition, or lifespan in people. The benefits, when they appear at all, are small and inconsistent. These are promising research compounds, not proven anti-aging tools, and anyone selling them as the latter is getting ahead of the evidence.
